Contact form section
The Contact form section adds a form with your own fields to a Kingdom page; messages go to your store's customer email address.
Applies to: Kingdom 6.2.0 and later. Added in: Kingdom 2.0.0. Last checked: 2026-10-13 against Kingdom 6.2.0.
The Contact form section adds a form to a Kingdom page with the fields you choose. Shopify emails every message to your store, with all the fields the customer filled in.
Where to find it: select Add section in the theme editor, then choose Contact form (Add section category Forms). The bundled contact page template already contains it, see Regular page.
Form fields
The form is built from blocks, shown in the order you drag them. The section holds up to 12 blocks.
- Name & email adds the Name and Email fields side by side. Both are required. Use it once, so you know who wrote and where to reply. A signed-in customer finds both fields already filled.
- Form field adds one more field. Label is the text shown inside the empty field and the field's name in the email. Type is Single line for short answers or Multi line for a message box. Turn on Required to stop the form from sending while the field is empty.
- Submit button adds the send button; Label sets its text (default "Submit"). Use it once, as the last block.
- Heading shows a title above the form (default "Drop us a line"). Heading tag sets its HTML tag, see General section settings.
After a message is sent, the form shows "Thanks for contacting us! We'll get back to you as soon as possible." Change that text in Shopify admin → Online Store → Themes → … → Edit default theme content.
Setting up a contact page
Create the page
In Shopify admin → Online Store → Pages, select Add page, give it a title such as "Contact", and set Theme template to contact. Save.
Add the fields
In the theme editor, open that page and select the Contact form section. Keep the Name & email block, then add a Form field for each question, for example "Phone" (Single line) and "Message" (Multi line, Required on). Select Save.
Link the page and test it
Add the page to a menu in Shopify admin → Content → Menus. Send a test message from the storefront and check your inbox.
Make sure that all of your fields have unique labels. Kingdom uses the label as the field's name in the email, so two fields with the same label send only one answer.
Where do contact form messages go?
To your store's sender email address. Shopify, not Kingdom, sends the message. Change the address in Shopify admin → Settings → Notifications → Sender email. Check your spam folder if a test message does not arrive. Customers do not get a copy.
Why are two of my fields merged in the email?
Because the two Form field blocks have the same Label, or labels that differ only in capitals or punctuation, such as "Phone" and "phone". Kingdom turns each label into a field name, and matching names overwrite each other. Give each field a distinct label.
Can I add a dropdown, checkboxes or a file upload?
No. Kingdom's Form field has two types, Single line and Multi line. Kingdom does not support other field types. Use a form app from the Shopify App Store, placed in an Apps section.
