Contact Form section

The Contact Form section builds a contact form in Split from fields you add, with an optional Google map, and sends messages to your store email.

Applies to: Split 5.1.0 and later. Added in: Split 4.0.0. Last checked: 2026-10-13 against Split 5.1.0.

The Contact Form section builds a contact form from blocks: a title, text, the name and email fields, your own fields and a submit button. Messages are sent by Shopify to your store email. The heading sits in the left half of the screen, with an optional Google map behind it, and the form fills the right half.

Where to find it: select Add section in the theme editor, then choose Contact Form.

Building the form

A new Contact Form section starts with a Title, the Name & email fields, a Message field and the Submit button. Reorder the blocks in the theme editor sidebar to change the order of the form. The section takes up to 8 blocks.

  • Title (section setting) is the large heading in the left half.
  • Heading tag sets its HTML level for search engines; it does not change its size.

The Cuber preset uses this section in its contact page template. See Regular page for how to give a page that template.

Blocks

Title

  • Text is a small heading above the fields, for example "Write us a message". One per section.

Text

  • Text adds a paragraph, for example your address and phone number. You can add several.

Form field

  • Label is the field's placeholder and its name in the email you receive. Every field needs a different label.
  • Type chooses Single line (a short answer) or Multi line (a message box).
  • Required stops the form from being sent while the field is empty. Off unless you turn it on.

Name & email

Adds the Name and Email fields side by side. Both are required, and they are filled in for customers who are logged in. No settings; one per section.

Submit button

Adds the "Submit" button. No settings; one per section. Without it, customers cannot send the form.

Make sure that all of your fields have unique labels. Two Form fields with the same label are sent as one field, so one of the answers is lost.

Adding a map

The map fills the left half behind the heading. It uses Google Maps and needs your own API key.

  • Enable map turns the map on.
  • Map address is the address Google puts the pin on. The map loads only when this field is filled.
  • Google Maps API key is the key from your Google Cloud project. The Contact Form section has its own field: the key from a Map section is not shared.
  • Map style chooses a light grey or a black map.
  • Map custom pin replaces the default pin with your own image, shown at 60 x 60 px. Upload a 240 x 240 px transparent PNG.

Customers who select the pin see the address and an "Open in Google Maps" link. To create the key, follow Setting up the Google Maps API key on the Map section page.

Where do contact form messages go?

Shopify sends them to your store's sender email, set in Shopify admin → Settings → Notifications. The theme only builds the form; delivery, spam protection and the email itself are handled by Shopify. Each message lists the name, email and every Form field by its label.

Why does my map not show?

Usually because the address or the key is missing or wrong. Check these, in order:

  • Map address is empty: the map does not load at all.
  • Google Maps API key is empty, wrong or restricted to other domains: Google shows its own error box instead of the map.
  • Map address cannot be found by Google: the map stays on an empty stretch of ocean with no pin. Write the full address with the city and country.
  • The Maps JavaScript API or the Geocoding API is not turned on in your Google Cloud project, or the project has no billing account.

The Contact Form map has no fallback image. If the map cannot load, use the Map section, which shows its Image instead.

Why does the form show "Thanks for contacting us"?

That is the confirmation Shopify shows after a message is sent. The text is theme content: change it in Shopify admin → Online Store → Themes → … → Edit default theme content.
